I admit it:  The only reason this story is here is because of the video the follows after the click.  The operator of a light rail train that jumped the tracks and ran halfway up an escalator at Chicago's O'Hare International Airport admitted that she dozed off and did not wake up until the crash.  And it was caught on video:

So this guy just comes up the escalator early on Monday morning, and because it’s Chicago, he’s probably asking, “Hey!  When does da deep dish pizza place open over der?  Go Cubbies!” or something like that because it’s Chicago.  And in the background, you can see the lights of the train approaching, and approaching, and not slowing down.

BAM, the train jumps the terminus at just over 40 kiloemters per hour and goes up the escalator; and yes, the security guard doesn’t grab the weary traveler and pull him to safety as much as he saves his own arse.  All captured by the security camera.  33 people were hurt, none critically.  An automatic trip was supposed to have automatically put on the brakes more than 10 meters before the terminus, but that apparently didn’t happen.

The driver admits she fell asleep, and the penalty might be dismissal.  Federal investigators say it’s her second time asleep at the switch.  A month ago, the unidentified operator admitted she dozed off when she drove a train slightly past the platform at station at which she was supposed to stop, no one hurt in that earlier incident.
